Consultation and updating of location and contact details on the SIP health card, and choice of language
What is
Consult and/or modify in the SIP (Population Information System) health card system the habitual residence, postal address, contact telephone numbers, email address and, if the card holder is hearing impaired, choose the desired means of communication so that health personnel can contact the card holder. As well as to choose the language to use for communicating with the administration.
The contact telephone numbers can be:
·CONTACT MOBILE TELEPHONE: This is the mobile phone number of the SIP holder and the one that will be used by the Conselleria for Health to communicate with the person (by phone call or text message).
·ALTERNATIVE TELEPHONE: In case of not being able to contact the mobile or landline, this alternative telephone number will be used.
·LANDLINE TELEPHONE: This is the landline telephone of the dwelling and of the Cohabiting Family Unit.
·FOREIGN TELEPHONE: This is the holder's mobile phone if it is a foreign mobile phone.
The communication methods for hearing-impaired people are:
·Communication via SVisual Platform: If the card holder wishes to communicate using the SVisual platform. (Video interpretation of sign language).
·Video Call Communication: If the card holder wishes to communicate by video call. The video call is complemented by the possibility of using online chat for the card holder, as well as simultaneous subtitling in future versions.
·Hearing loss with telephone communication: If the card holder wishes to communicate by voice telephone call.
If the card holder's address changes, based on the Health Map, the card holder may be assigned to a different Health Centre than the one currently assigned. Automatic assignment to the new Centre is possible if indicated. If the new address belongs to another Health Centre, and if the card holder has explicitly indicated in the Procedure that they do not want the Centre they already have to be changed, the card holder must request a Free Choice in the chosen Centre, maintaining, for the time being, the Centre they had.
The card holder may be asked for an identification documentation as deemed necessary by the operator attending them at any Health Centre.
The address entered can be temporary or permanent. If temporary is indicated, the card holder will have to choose the estimated number of months they will reside at that new address (between 1 and 6). After that time, if the card holder is still registered, the system will return them to their previous address, health centre and doctor.
