Consult and/or modify in the SIP (Population Information System) health card system the habitual residence, postal address, contact telephone numbers, email address and, if the card holder is hearing impaired, choose the desired means of communication so that health personnel can contact the card holder. As well as to choose the language to use for communicating with the administration. The contact telephone numbers can be: ·CONTACT MOBILE TELEPHONE: This is the mobile phone number of the SIP holder and the one that will be used by the Conselleria for Health to communicate with the person (by phone call or text message). ·ALTERNATIVE TELEPHONE: In case of not being able to contact the mobile or landline, this alternative telephone number will be used. ·LANDLINE TELEPHONE: This is the landline telephone of the dwelling and of the Cohabiting Family Unit. ·FOREIGN TELEPHONE: This is the holder's mobile phone if it is a foreign mobile phone. The communication methods for hearing-impaired people are: ·Communication via SVisual Platform: If the card holder wishes to communicate using the SVisual platform. (Video interpretation of sign language). ·Video Call Communication: If the card holder wishes to communicate by video call. The video call is complemented by the possibility of using online chat for the card holder, as well as simultaneous subtitling in future versions. ·Hearing loss with telephone communication: If the card holder wishes to communicate by voice telephone call.

If the card holder's address changes, based on the Health Map, the card holder may be assigned to a different Health Centre than the one currently assigned. Automatic assignment to the new Centre is possible if indicated. If the new address belongs to another Health Centre, and if the card holder has explicitly indicated in the Procedure that they do not want the Centre they already have to be changed, the card holder must request a Free Choice in the chosen Centre, maintaining, for the time being, the Centre they had. The card holder may be asked for an identification documentation as deemed necessary by the operator attending them at any Health Centre. The address entered can be temporary or permanent. If temporary is indicated, the card holder will have to choose the estimated number of months they will reside at that new address (between 1 and 6). After that time, if the card holder is still registered, the system will return them to their previous address, health centre and doctor.

Telematics
Individuals obliged to interact with the Generalitat through electronic means, under the terms of Article 14 of Law 39/2015, of 1 October, on Common Administrative Procedures, as well as applicants who, not being obliged to do so, choose to do so, may submit their applications online through the Generalitat's digital portal. 1.- The online submission of the application and the required documentation will be carried out THROUGH THE CORRESPONDING ONLINE PROCEDURE, whose direct link has just been indicated. For this purpose, the applicant must have a recognised or qualified and advanced electronic signature system based on recognised or qualified electronic certificates for electronic signatures issued by providers included in the "Trusted List of Certification Service Providers". These systems include the following: -The DNIe (electronic ID), certificates issued by the FNMT (Fábrica Nacional de Moneda y Timbre) and certificates for legal persons, public employees or individuals issued by the ACCV (Autoritat de Certificació de la C. Valenciana). -And the State's key-signature system only for natural persons (therefore, legal persons are excluded: companies, municipalities, organisations, etc.). More information about the systems admitted in the Generalitat's Online Portal can be found on: https://sede.gva.es/es/web/sede_electronica/sede_certificados 2.- In order to carry out Generalitat Administration procedures online while acting on behalf of another person (natural or legal), representatives must first register in the REPRESENTATIVES REGISTER held by the ACCV, or in the ELECTRONIC POWER OF ATTORNEYS REGISTER held by the General State Administration, whose respective information is available on the following websites: - http://www.gva.es/es/inicio/procedimientos?id_proc=17168&version=amp - And http://www.accv.es/ciudadanos/registro-de-representantes/ Without this prior registration, in addition to not being accredited as a representative, it will not be possible for the representative acting on behalf of the other person to receive anything in their citizen's folder. 3- By clicking on the direct link indicated in "How to process online" or on the link "PROCESS WITH CERTIFICATE" (in red at the beginning of this procedure), you will access the online procedure which will allow you to fill in the initial web form, attach the application and other completed and digitally signed documentation that you must provide and submit it online, obtaining the corresponding registration receipt.
